First Time Clients

So we can better get to know you and your animals, we ask that your print off this form and send it to us before our first visit. We will have copies of this form available during our first visit if you have not filled it out yet. The form can be found here.

Appointment Scheduling

Here at MVC we are by appointment only, so calling our office staff ahead of time for appointments is required.

Payment

Unless prior arrangement has been made, payment is due as soon as services are rendered. Monterey Veterinary Center accepts payments in multiple forms including:

Care Credit-Care Credit is a credit card used to pay for medical or veterinary expenses. Please see this site for more information.

Cancelation Policy

We have implemented a no call/no show policy to ensure we are able to use our limited time efficiently. If a client no call/no shows there is a fee that must be paid in full prior to coming in for the next appointment. There are discretionary exceptions for extreme circumstances, but that is a case by case decision.
